Our hours
are Monday thru Thursday 8:30 AM to 5:00 PM. Fridays are
8:30 AM - 4:00 PM.
When you
arrive for your visit, you will be asked to answer a
brief questionnaire and we will obtain a current copy of
your insurance coverage. Please bring a list of your
medications and any allergies to medication you may
have.
Please
allow at least 1½ hours for a complete eye examination
by Dr. Goffman. This time allows for a comprehensive
examination of your eye and its anatomy, including
dilation to inspect the health of your retina and optic
nerve.
If you
want to save time please fill out these forms and bring
them with you. You must have the Adobe
Acrobat Reader installed on your computer to
